Good Faith Exam

As per Arizona Board of Nursing (BON) regulations, a doctor's order is now required to receive Level 2 and Level 3 procedures, including Microneedling, PRP, Neuromodulators, and Fillers. To comply with these laws, we now conduct a Good Faith Exam via telehealth with a licensed provider to assess your needs and determine if the treatment is appropriate.

 

What is a Good Faith Exam?
A Good Faith Exam is an essential consultation where a medical professional evaluates your medical history, overall health, and suitability for treatments. This exam ensures that procedures are safe and tailored to your needs, minimizing risks and promoting optimal results.

 

How It Works:
You will have a telehealth consultation with an experienced medical provider, who will:

• Review your health and medical history

• Answer any questions you may have

• Determine if the treatment is safe and appropriate for you

 

What’s Included:

• Before the consultation, you'll complete a brief questionnaire covering your medical history, allergies, past treatments, and current health status.

• The exam will include a medical evaluation, discussion of any concerns, and a treatment plan outlining potential procedures, their benefits, risks, and alternatives.

• Each exam typically lasts about 15mins, depending on treatment complexity.

 

Fee & Application:

The telehealth assessment costs $30, however Bella BeautyRN will cover the fee. It is required for new clients, annual assessments, or any changes in health.
